Atlantic Canada, Bermuda and the Caribbean

The northern edge of the offset switches with the seasons; the tropical part mostly does not. Places that keep UTC−4 as their standard offset include: Atlantic Canada — Halifax, Moncton, Saint John, Charlottetown, and most of Labrador.

Bermuda — Hamilton, which moves to UTC−3 in summer on the North American dates. Puerto Rico and the US Virgin Islands — San Juan and Charlotte Amalie, year-round. Greater Antilles — Santo Domingo in the Dominican Republic.

Lesser Antilles — Bridgetown, Port of Spain, Fort-de-France, Basse-Terre, Willemstad, Oranjestad. Atlantic Canada and Bermuda observe daylight saving. The Caribbean territories on the list do not, which is why San Juan matches Halifax in winter and falls an hour behind it in summer.

South America on UTC−4

The continental share is substantial and largely fixed: Bolivia (La Paz, Santa Cruz), Venezuela (Caracas, Maracaibo), Guyana (Georgetown), and a wide band of western Brazil including Manaus, Cuiabá, Porto Velho and Boa Vista. None of those change clocks.

Chile is the exception that moves in the opposite direction. Its mainland standard offset is UTC−4, used through the southern winter, with a shift to UTC−3 for the southern summer.

Two hemispheres, one offset

Around March and April, UTC−4 changes hands: North Atlantic clocks leave it as they spring forward, and Chilean clocks arrive as they fall back. That handover is the thing to watch if your work spans both hemispheres. Between roughly March and April, and again between September and November, the relationship between Halifax and Santiago changes twice within a few weeks, and neither city considers itself the one that moved. Meanwhile Caracas, La Paz and San Juan sit still through the whole performance.

Some quick anchors: UTC−4 is one hour ahead of US Eastern Standard Time in winter and level with Eastern Daylight Time in summer. It is four hours behind UTC, and five hours behind London when London is on summer time.
